At Comprehensive Behavioral Care, we value our providers. As our partner, we want to make it as easy as possible for you, the provider, to deliver quality care to our members. You will find a wealth of information here including an up-to-date provider manual, policies and procedures; see also our bulletins section [link on left].

We believe communication is key to a successful partnership. Provider Services Representatives are available to answer your questions and concerns during normal business hours via our toll-free number - 1-800-458-6139.

How to Join the Network
Joining our network begins with submission of the provider selection form. After the monthly Provider Selection meeting, you will be notified of the decision. Based on network need, contract & credentialing documents will be included with an invitation. Once all documents are received the verification process begins and usually takes about 6-8 weeks. Upon approval, you will receive a welcome letter. Practitioners must be contracted and credentialed prior to providing services to our members.

National Provider Identifier Number (NPI) is required as of May 23, 2007
You must notify Comprehensive Behavioral Care of your NPI number so we may enter it into our system for identification, electronic claims payment, and financial transactions as outlined under HIPAA. You may send us your NPI number by faxing it on your letter head to (813) 288-4844 Attention: Provider Services, by entering your last name, first name, licensure, and demographics under Contact Us or by calling our Provider line at (800) 458-6139.
